About the Discoverer
Sota Omoigui M.D. is CEO of State-of-the-Art Technologies Inc and Medical Director of the L.A. Pain Clinic in Hawthorne, California. He is board certified in Anesthesia with subspecialty certification in Pain Medicine. He has served as an adviser to the United States, FDA Advisory Committee on Anesthetics and Life Support Devices.
He has 1037 plus Google Scholar citations for his scientific research work.
He is a Recipient of the US FDA Advisory Committee Service Award, in recognition of his distinguished service to the people of the United States of America.
Since the dawn of time, 300,000 years ago, Mankind has tried to understand the origins of pain. From Athenian philosopher Plato (c. 428 to 347 B.C.) to the 1644 pain theory , by the French philosopher Renee Descartes to the 1965 gate control theory by Melzack and Wall. None of these theories have withstood the test of time.
In his seminal book titled: The Biochemical Origin of Pain and published in 2002, Dr Sota Omoigui proposed his theoretical law that the origin of pain is due to inflammation and the inflammatory response. His subsequent publication of his theory as a journal article in 2007 has been cited as of this date by 493 medical publications. In his 2002 theory, he demonstrated a clear association between migraine and the release of inflammatory mediator calcitonin gene-related peptide (CGRP) and substance P (SP). In 2019, seventeen years later, the US FDA approved members of a new class of drugs specifically designed to treat migraine by targeting calcitonin gene-related peptide (CGRP). Those drugs have been the greatest advance in the treatment of migraine.

Good afternoon Dr Sota. I am in the United States.My son is four years old with sickle cell anemia.
He was diagnosed through the new born screening. The drs here have had him on hydroxy urea medication and penicillin prophylaxis since he was about 18 months. While he hasn’t complained about pain much, he has however been hospitalized several times since birth for mainly acute chest / pneumonia.
Last year he was hospitalized more than six times for acute chest.

Good day Dr. Sota. May God bless you real good. Pls, my son Haa been using the oxygen tank since October last year n has successfully combated 2 crises. Initially, he was skeptical about using it. You may remember that some time ago, I informed you that I'd purchased a POC and that I was going to spend time to observe its effectiveness in the prevention/ abortion of a sickle cell crisis.
Since then, I've been quiet, just to enable me have an honest assessment.

Patients recorded daily pain that did not progress to severe crisis, especially when oxygen was applied early (within 30 minutes).
Reported outcomes:
• “Pain stopped within 10–20 minutes”
• “Crisis aborted before needing hospital”
• “Able to sleep through the night when oxygen was used”
In contrast, non-oxygen patients often reported:
• Crisis lasting multiple hours
• Slow aborting of pain
• High need for NSAIDs / opioids
Prevention and Clinical Management of Sickle Cell Crisis
Prevention
When there is any stressor during the day, as listed below, the person should use oxygen as described:
Inhale oxygen for 10 minutes to 2 hours before sleep or use the oxygen for as long as needed for part or the entire duration of sleep.
Flow rate:
Children 0.5-1.5 lpm
Adults - 1-2 lpm
Note: Sleep is the most dangerous time as the low oxygen environment will tip the person into a crisis:
Physical stress: eg from physical activity
Intellectual stress eg preparing for exams, writing papers
Environmental stress eg cold environment, high altitude, air travel
Physiological stress eg during sleep, excess heat, dehydration, pain, emotional stress, infection, malaria.
Note: Infection and malaria, bone marrow suppression from toxic doses of Hydroxyurea or hemolytic crisis from antibodies in improperly cross matched blood transfusions, also need to be prevented and timely treated. Otherwise there will be more reversible sickle cells than oxygen can restore.
Treatment
Home Oxygen used to stop a crisis in the golden half hour (30 mins) of onset of pain. If the crisis does not stop, please proceed to the hospital as you may be having an infection, malaria or hemolysis
Hospital Pain Control
Proper and rapid control of pain is critical to alleviate suffering and more importantly to prevent acute chest pain syndrome resulting from chest splinting and shallow breathing leading to low oxygenation. Acute chest syndrome has 25% mortality. Pain Control is achieved by the use of pure opioids like Morphine, Pethidine, Hydromorphone and potent analgesics like Ketamine. These should be combined with anti-inflammatory drugs like Diclofenac or Ketorolac. Weak opioids like Pentazocine or Tramadol should never be used to treat the pain of a sickle cell crisis. Pentazocine also has psychiatric side effects and is banned in the United States.
Hospital Fluid Management
Sickle Cell patients have increased risk of pulmonary edema due to diastolic dysfunction. Monitor fluid input and output. Also include the amount of blood transfused. Pulmonary edema can be confused with acute chest syndrome and both have a high mortality rate.
